If you wish to register with the practice you will be asked to complete a GP registration form and new patient questionnaire form. You can download these from the website or call into the practice to collect a new patient pack.
Your forms will be reviewed by a GP and you will be advised if you are required to attend a registration appointment and with whom. This may be a GP, chronic disease nurse or a treatment room assistant. Some patients may not be required to attend a registration appointment.
Patients are advised to the read the NHS counter fraud declaration before signing the registration form.
Acceptance on to the list is as at the discretion of the GP. We may also ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
Once you’ve been accepted as a patient, your medical records will be transferred to the Practice. Your medical records may take up to several weeks to arrive at the Practice.
If you default from your new patient appointment and do not inform the reception staff you may be refused acceptance on to the Practice list.
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.
